Illinois School Health Requirements

NOTICE:  Illinois children entering Kindergarten in the Fall of 2002 will be required to receive a vaccination against chicken pox (varicella) or provide proof of having had the disease.  State law does provide exemptions from the immunization requirements for religious or medical reasons.  

I.    Physical Examination Required for Those Entering:
            -preschool programs
            -kindergarten
            -grade 5
            -grade 9

II.  Dental Examination:
            -Required for students entering Kindergarten, Second, and Sixth grades. (Click here for a printable copy of this form.)

III.    Immunizations:

            DPT:
                -Basic series required
                -Booster required after age 4

            DT:
                -Booster required every 10 years
                -Usually at entry to grade 9

            Polio:
                -Basic series required
                -Booster required after age 4

            MMR:
                -2 required
                    - #1 after age 12 months
                    - #2 usually after age 4 (but must be no less than one month after #1)

            HiB (Hemophilus influenza type B):
                -required for all students 2 years of age or older in a school operated program below kindergarten level

            Hepatitis B (series of 3):
                -required for all students 2 years of age or older in a school operated program below kindergarten level
                -required for all students entering grade 5 in the Fall of 2002

            Lead Assessment:
                -required for children through age 6
