Medication Guidelines

 

1. All medication brought to school must be in the unopened/sealed, original store-bought package.

2.  The package must be of a small size - no more than 24 tablets per box or 50 tablets per bottle or 2-4 fluid oz. liquid.  The reason for this is that there is limited space in our locked medicine cabinet.

3.  A School Medication Form must be completed and signed by both physician and parent for all prescription and nonprescription medications.  Click here to print a copy of the School Medication Form.

4.  A parent must pick up all medication from school.  Medication WILL NOT be sent home with a student.

 


 

6th Grade Physical Beginning with the 2008-09 school year, students entering the 6th grade are required by the State of Illinois to have a physical examination.   Information regarding this requirement is sent home with all Prairie Central students (or included as an insert in the report card) at the end of their 5th grade school year.  Students who attended the U.E. in 5th grade (2007-08 school year) will not need another physical for the 6th grade.  

The student's physical should be obtained prior to registering a 6th grade student.  Immunizations must also be up-to-date.  Click here to print a State of Illinois 6th grade physical form.

The physical should be presented at the time of registration along with the immunization record completed.  The Health History section must also be completed and signed by the parent or legal guardian.  Those students not in compliance with the Illinois Health Requirements will be excluded from attending school.  

 


 

6th Grade Dental Exam Students entering the 6th grade are required by the State of Illinois to have a dental exam.   Information regarding this requirement is sent home with all Prairie Central students (or included as an insert in the report card) at the end of their 5th grade school year.  Click here to print a State of Illinois 6th grade dental exam form.

The dental exam should be presented at the time of registration.  Those students not in compliance with the Illinois Health Requirements will be excluded from attending school.

 


 

KidCare Health Insurance for Illinois Children

What is KidCare?
It's a state program that offers health care coverage to children and pregnant women and help in paying premiums of employer or private health insurance plans.

Who is eligible for KidCare?
Children through age 18 who are Illinois residents, are U.S. citizens or permanent legal residents, and meet income requirements are eligible.  Pregnant women of any age who are Illinois residents and meet the income requirements are eligible.

What are the income requirements?
Income requirements vary by KidCare Plan and are based on family size.  Payments made for childcare expenses or child support will be subtracted from income.

What will KidCare cost me?
KidCare services are available at no cost or at low cost.  How much you pay, if anything, depends on your family income.

What health services does KidCare cover?
Services include doctor and nursing care, immunizations and preventative care, hospital and clinic care, laboratory tests and x-rays, prescription drungs, medical equipment and supplies, medical transportation, dental care, eye care, psychiatric care, podiatry, chiropractic care, physical therapy, mental health and substance abuse services.  Pregnant women also receive prenatal care and other medical services.

What if my family has coverage through an employer or private plan?
Families who have employer or private health insurance can get help from KidCare Rebate to pay their insurance premiums if their family income falls between certain monthly income standards.  Under KidCare Rebate families get the services provided by their employer or private health insurance plan.
